Custom Evaluation Forms for Call QA: A Configuration Guide

Editorial illustration: Custom Evaluation Forms for Call QA: A Configuration Guide

A custom evaluation form turns a quality standard into fields that reviewers and software can apply. The form should make the question, evidence and result clear enough that someone can inspect a disputed score later.

Start with a methodology your team understands. The QA scorecard design guide covers what to measure and why. This article focuses on configuring that standard for the right workflow and keeping changes controlled.

Define More Than a Label and a Weight

“Product knowledge: 20 points” leaves too much unspecified. A useful form stores a criterion identifier, a precise question, the applicable call types, the expected evidence and the rule for each result.

For example, a product-accuracy criterion might ask whether the agent's answer matched the policy effective at the time of the call. Its evidence includes the statement and that policy version. If the applicable policy is missing, the form should allow an unresolved assessment.

Keep Sections Tied to the Call's Purpose

Sales qualification may need discovery, fit, accurate answers and next-step confirmation. Support may need verification, diagnosis, policy accuracy, action completion and handoff. Use shared fields where the meaning is genuinely the same.

Add actor-specific checks for AI callers, such as whether a tool result supported a success statement or whether a failed transfer used the approved fallback. Human coaching criteria should describe behavior the person could reasonably control.

Avoid forcing a fixed number of sections onto every team. A short form with clear evidence can be more useful than a detailed form filled with overlapping judgments.

Route Forms Using Trusted Context

Select the form from approved client, queue, campaign and workflow configuration. A conversation classifier can suggest a mismatch, but it should not silently choose easier criteria for a difficult call.

For mixed-intent calls, define whether to use a combined form, separate relevant sections or human review. Keep the routing decision in the record so an evaluator can explain why a criterion applied.

The sales versus support comparison gives examples of the differences.

Map Evidence Before Turning on Automatic Scoring

For each field, identify where the evidence comes from and what happens if it is absent. Spoken agreement may be enough for one question; a confirmed booking requires a connected-system result.

Do not infer action completion from a polite closing sentence. Equally, do not mark a person down when a missing integration prevents the system from seeing an action that may have happened. Route the uncertainty to review and show the evidence gap.

Define scoring arithmetic separately from language interpretation. The evaluator supplies structured findings; application code calculates the total according to the approved rule.

Version and Test Form Changes

Keep published forms immutable or retain their exact historical versions. Draft a change, test it on representative calls and review differences before assigning it to new work.

If a policy changes, record its effective time. Do not rescore old calls against new expectations without identifying the exercise as a separate re-evaluation. Preserve the original result for auditability.

Test boundary cases: no objection raised, incomplete opening audio, uncertain speaker attribution, a failed action and a successful escalation. These reveal whether the form handles more than ideal conversations.

Confirm the Actual Configuration Experience

When evaluating custom call QA, ask how forms are created, approved, assigned and exported. Do not assume a drag-and-drop builder, spreadsheet import or a particular report format from the phrase “custom scorecards.” Confirm what the specific deployment supports.

Bring your current form and one disputed call. The useful demonstration is whether the criterion, evidence and correction can be represented clearly.

Frequently Asked Questions

Can an existing spreadsheet be reused?

Its criteria can be reviewed and mapped into a structured form. File import support is a separate capability to verify, and ambiguous spreadsheet wording still needs clarification.

Should compliance be another weighted section?

Some policy checks can contribute to quality scoring, but critical requirements need an explicit rule that cannot be averaged away.
